In order for a midwife to enter into private practice as a midwife she/he must first notify the intention to do so to the Department of Health.

The statutory requirement to report the intention to enter private practice as a midwife is specified in Regulation 3 of the Health (Notifications by Midwives) Regulations 1994.
Responsibility for notification
The midwife intending to enter into private practice.
Who must be notified
Under the Health Act 1911, the Executive Director, Public Health must be notified. Notifications are sent to the Nursing Midwifery Office.
How to notify
Complete the notification of intention to enter into private practice as a midwife form.
